Established in 1985, Community Hospice Foundation is a charitable, non-profit organization dedicated to providing financial assistance that helps improve the quality of life for people who have been diagnosed with a terminal illness.
As the philanthropic arm of ThedaCare At Home’s hospice program, our foundation is devoted solely to funding compassionate care programs for people during their end-of-life journey, as well as offering support services for their families.
Hospice care doesn’t exist in one location—people can receive hospice care in their home, at an assisted living or skilled nursing facility, or in a hospice residence.
The Community Hospice Foundation contributions ensure that wherever patients choose to receive hospice care, their individual needs—as well as the needs of their family and caregivers—are met with compassion and dignity.
