Ways to Give
Through the generous donations of grateful friends and families of patients, as well as corporate and organizational donors, Community Hospice Foundation has been able to provide a variety of programs and services using our foundation funds.
Helping Hands Fund
In addition to the general support our foundation supplies to the facility, this program provides financial assistance to residents of Cherry Meadows, the hospice residence located within Peabody Manor in Appleton.
Giving additional comfort and support, music therapy helps us offer more holistic care for patients, and allows for a different way of communication and expression
We provide the funds needed to support the pet therapy program—which offers a special connection for many patients—including the training required for volunteers and their dogs.
Every Life Has a Story Video Program
Hospice volunteers work with patients and their families to record the patient’s memories and wishes for loved ones on video. The footage is transferred to a DVD that the family can keep and cherish after their loved one has passed.
Lifetime Legacy Scrapbooking Program
Volunteers assist family members and patients in creating beautiful scrapbooks that celebrate the patient’s life and experiences. Our foundation helps fund the supplies needed to create these keepsakes.
Hospice Hopes and Dreams Fund
One of the most rewarding programs we support, this fund lets the ThedaCare At Home hospice program make a "last wish" come true for a hospice patient or their family.
ThedaCare At Home Studio Residence
This nine-suite hospice residence is the first of its kind in Wisconsin. The facility is designed to meet the needs of hospice patients who can live independently, but still require meal services, housekeeping assistance, and medical support.
