Areas to Support
Through the Community Hospice Foundation you are able to choose from several ways in which your
gift(s) may be directed to support the hospice care provided by ThedaCare At Home™. You can designate your gift to a particular need, or provide the Foundation with the authority to address the
area(s) of greatest need. The Foundation offers two kinds of funds.
- Restricted Funds direct your donation to a particular need. To learn more about these opportunities, please see the list below.
- Endowed Named Funds are permanent named funds that carry on into perpetuity, providing support now and for generations to come. With a gift of $10,000 or more, over three-to-five years, donors may establish endowed named funds to be administered in support of a particular hospice program or initiative. Community Hospice Foundation Funds
Area of Greatest Need
Giving to Community Hospice Foundation’s Area of Greatest Need is one of the most valuable gifts you can make to support patients and their families. Contributions to this Fund will be put to valuable use where the need is most essential in order to ensure that the highest quality of care is delivered to anyone in need.
Helping Hands Fund
This fund serves patients who reside at Cherry Meadows and require financial assistance. The funds, which are allocated on a case-by-case basis, assist patients who are under-insured or have limited capacity to pay.
“Home Care” Fund
Donations to this fund provide assistance to hospice patients who are in need of financial support, but wish to remain in their home during their hospice journey.
Hospice Hopes & Dreams Fund
This fund provides financial assistance to fulfill some of the last wishes, dreams or hopes of patients during their end-of-life journey. These wishes may include traveling to see family, attending a play or sporting event, or the enjoyment of a massage.
ThedaCare At Home™ recognizes the positive influence that animals can have on hospice patients. Specially trained dogs provide entertainment and connect the patient to the outside world, and are a great distraction from a patient’s physical, emotional or spiritual pain. It is no surprise that pets are special and can connect with the human spirit sometimes more effectively than a caregiver. Donations to this fund will enable the Community Hospice Foundation to support a pet therapy training program for dogs and their owners. When a dog completes this program they will be qualified to interact with hospice patients in any setting, including private homes.
Waupaca Community Hospice Hopes & Dreams Fund
This fund provides financial assistance to help fulfill some of the last wishes, dreams or hopes of patients in the Waupaca community during their end-of-life journey. These wishes may include traveling to see family, attending a play or sporting event, or other personal wish.
Waupaca Community Fund
Giving to the Waupaca Community Fund is one of the most valuable gifts you can make to support patients and their families in Waupaca and the surrounding communities. Contributions to this Fund will be put to valuable use where the need is most required within.
