Making a gift now or in the near future has an immediate impact and is easy to do!  If you wish to support the Community Hospice Foundation with a donation today, we can help you give the gift you wish and in a way that results in tax or other benefits. There are a number of opportunities available to you.

Gifts of Cash

Cash gifts make an immediate impact on patient programs, community education, and special initiatives. Gifts may be made in memory or in honor of a friend or loved one. A gift of cash is the simplest, most direct, and most popular way to support the Community Hospice Foundation.  The amount of your gift can be deducted from taxable income to the full extent allowed by law.

If you wish to make a gift of cash, please fill out a donation form. Please make your check payable to the Community Hospice Foundation.

Pledges

You can pledge a gift to the Foundation and pay it over a period of up to five-years and on a schedule convenient for you. For more information, call or e-mail the Foundation at (920) 738-6503 or CommunityHospiceFoundation@thedacare.org.

Tribute Gifts

Some of the most meaningful gifts we receive are those given in memory or in honor of a loved one. 

When a loved one passes, the Community Hospice Foundation will help you honor and celebrate their life. Please tell us whom you wish to remember, and the name and address of the person we should notify of your gift. 

You may wish to honor friends or loved ones with a gift to the Community Hospice Foundation in their name, or on the occasion of a special event, such as a birthday or anniversary, wedding or retirement.  It’s the perfect way to support the Foundation while paying tribute to someone special in your life. Be sure to tell us who you are honoring, and provide that person’s name and address so that we can notify them of your gift in their honor.

All tribute gifts will be acknowledged with a certificate to the person you designate.  We will not share the amount of your gift, just the news of your thoughtfulness.

Appreciated Stocks or Bonds

You can make a direct gift to the Foundation – and gain significant tax advantages – with a gift of appreciated stocks and/or bonds through your broker, banker, or other financial consultant.

Appreciated Real Estate

You may receive the full fair-market-value charitable deduction by donating appreciated real estate such as vacation homes, farms, second homes, inherited property, or commercial and/or residential lots.  For more information, call or e-mail the Foundation at (920) 738-6503 or CommunityHospiceFoundation@thedacare.org.

Matching Gifts

To encourage philanthropic giving, many organizations will match their employees’ and retirees’ gifts.  The Community Hospice Foundation is an eligible non-profit organization that meets the guidelines of most matching gift programs.  Please check with your employer to see if they will match your charitable gift to the Community Hospice Foundation.

Endowed Named Funds

With a gift of $10,000 or more, payable over three-to-five years, donors may establish endowed named funds to be administered in support of a particular program or initiative. Endowed funds help ensure that important hospice programs will be sustained into perpetuity. Team Giving

We encourage our supporters to involve their family, friends, co-workers, employers and employees in raising funds for the Community Hospice Foundation by planning your own fundraising event.  These can be considered “Third Party Events” and are hosted by individuals and/or organizations external to the Community Hospice Foundation. When joining together as a team, there are many fun options to raise money for the Foundation.  Some of the more popular ideas are listed below.

Organize your own sporting event – golf outing, tennis match, bowling tournament, etc. – with proceeds directed to the Community Hospice Foundation.

Host a bake or craft sale with proceeds directed to the Community Hospice Foundation.

Rather than exchanging gifts at the holidays, funds can be collected and donated to the Community Hospice Foundation.

Ask your employer if employees can “pay” to wear jeans to the office or host “spare change war” among staff members with funds collected designated to support the Community Hospice Foundation.

Event Sponsorship

Show your dedication to the health of our communities by sponsoring one of our annual fundraising events.  Sponsoring a special event draws attention from the community to you or your company.

The amount of your sponsorship, less the value of goods or services received in return, is tax deductible.
