Community Health Action Team

ThedaCare Community Action Team

A diverse group of 25 leaders from all sectors of the community, including education, government, business, and non-profits and healthcare, comes together to understand local health issues.

Coming Together for a Healthier Community
Because health is primarily a product of factors outside of healthcare, health problems need to be addressed within the community, by partnering with people from all backgrounds and life experiences.

Exploring Local Health Issues
ThedaCare’s Community Health Action Team (CHAT) brings together diverse groups of people to study problems first-hand through "plunges," which are day-long events that gather people most affected by or involved in solving an issue. Whether we’re attempting to reduce poverty or improve senior health, community members share knowledge, learn from those at the heart of the issue, and develop solutions for change.
