Transitional Care

At New London Family Medical Center, we know that some patients no longer require hospitalization, but they aren’t quite ready to be at home on their own. That’s why we have our Transitional Care Unit (TCU). Our TCU provides services and care to patients who need extra help due to continuing medical needs such as wound care, Physical or Occupational Therapy, or IV antibiotic therapy.
