The staff of ThedaCare At Home becomes part of a patient's healthcare team and provides services to a patient to help them stay in their home. Home safety assessments, patient and family teaching, medication management, wound and ostomy care, post-operative care, and dietary counseling are used to manage chronic illnesses, cancer, cardiac, respiratory, and other medical conditions. We help coordinate care and provide ongoing progress reports to the patient, their family, physician, and all team members, helping to ensure continuity and effective delivery services.
For most clinical services, a physician refers a patient to our services. ThedaCare At Home and your physician work as a team to insure your health needs and goals are met. Each member of our team is highly trained and committed to high quality standards. Ongoing education ensures that our patients will receive the latest healthcare technologies and therapies. Clinicians are on-call 24-hours a day, 7 days a week for most services.
