Creating Healthier Communities

ThedaCare has taken its mission beyond the walls of its hospitals and clinics into the community. The Community Health Action Team (CHAT), comprised of a diverse group of community members, explores local health issues through “plunges,” or trips to community sites to learn first-hand about issues affecting our health. These plunges also create relationships among community resources and assets and point out opportunities for collaborative action.
