ThedaCare At Home is an integral part of a patient’s health care team. Our staff will assist a patient’s physician, nurse, discharge planner, social work and other hospital and service agency personnel in assessing your specific patient and family needs.
Close collaboration with all care providers ensures that the type of care planned for each patient is well coordinated and services are delivered in a timely manner. It also means that ThedaCare At Home personnel communicate regularly with the referring physician and/or other referral personnel.
ThedaCare At Home staff are specially trained in home and hospice care. We build on that training with a strong continuing education program so our patients have access to the latest technologies and therapies.
