Hospice Care

ThedaCare At Home™ provides compassionate care for people facing a life-limiting illness.  Hospice is a special health care program designed to assist people with an advanced disease and who have less than six months to live, when a curative approach to care is no longer appropriate. The focus of hospice care is to provide comfort with a team-oriented approach.  Hospice provides expert pain and symptom management, emotional and spiritual support tailored to the patient's and families individual needs and wishes.  Hospice believes everyone has the right to dignity and comfort and supports the unique journey of every patient and family.  Hospice will provide bereavement support to families for one year following the patient's death.

ThedaCare At Home offers Hospice services in the patient's own home, a nursing home, assisted living facilities, as well as our residential facility in Peabody Manor, Hospice At Cherry Meadows. Pain and symptom management is provided by meeting the physical, spiritual, social and emotional needs of patients facing end of life.

A team of professionals plan the patient's total care needs with the patient and family. They offer pain and symptom management, assistance with personal cares, hospice-related medical supplies, equipment, and medications, emotional and spiritual support, and grief counseling to support families for up to one year following the death of a loved one.

Insurance Coverage:  Hospice care services are covered under Medicare, Medicaid and most private insurance plans.

Caregivers:  Typically, a family member serves as the primary caregiver.  Members of the hospice team make regular, brief visits to provide additional care and support.  Hospice cannot provide extended care in the home.

The Medicare Hospice Benefit:  Medicare covers 100% of the care provided by hospice for the hospice diagnosis, and 95% of the cost for all medications related to the hospice diagnosis.  The Hospice benefit covers 100% of the medical equipment provided by hospice staff (such as wheelchair, oxygen, etc.)  Your regular Medicare benefit stays in place for non-hospice related illness or injuries.  Medicare also covers in-patient respite stays up to five days.  Acute inpatient care at Cherry Meadows or the Hospital for acute symptom management are covered on a day by day basis.  Medicare does not pay for room and board in a skilled nursing facility.

Staying at Cherry Meadows:  Hospice patients and families use Cherry Meadows in three ways:

  • Acute care: short term for pain and symptom management (covered by Medicare and Medicaid and some insurances.
  • Respite care: up to five days to relieve caregivers (covered by Medicare and Medicaid)
  • Residential care: this is typically private pay as Medicare, Medicaid and most insurance do not cover it.
