Clinical Contracts
The following requirements need to be met when a ThedaCare location is used as a student clinical site.
A. A current contract should be filed at the appropriate ThedaCare location after it has been
signed by representatives from the educational facility and ThedaCare. This contract
addresses:
1. Responsibilities of both parties;
2. Liability coverage for the students;
3. Adherence to hospital or site health requirements; and
4. Compliance with Caregiver Background Check Law
B. The educational facility has a responsibility to furnish ThedaCare with the following
documentation prior to the beginning of the clinical experience:
1. Student and faculty health information based on location requirements;
2. Verification of faculty and students' adherence to the state Caregiver Background Check
Law requirements.
3. Student clinical plan;
4. Documentation of adherence to other requirements.
a. CPR if required for the role for which they are preparing
b. Faculty licensure or certification documentation, if applicable.
c. Documentation that the student using a ThedaCare location site have information
regarding the Hazardous Communications, Safety, and Infection Control Program,
emergency response, and HIPAA overview.
d. Ongoing monitoring and evaluation of the student clinical process
