Thank you for your interest in ThedaStar's Ride Along Program.
Any personnel who meet the requirements may apply for the Ride Along Program by completing an application and sending it to ThedaStar at the address below. The completed application will be reviewed and you will be scheduled for a date that your choose.
Things you will need to submit with your application include:
- Completed application with 1st, 2nd, and 3rd choices of shift
- Copies of any Licensure or Certification
- Health Status Form completed (see link below)
- Copy of your State Criminal Background Check. (This will be an actual state document that your employer or service will have on hand. If your employer or service does not have this on file, please contact the State Department of Licensure for instructions on how to obtain this for yourself. This document, if your employer or service does not have this on file, will cost about $18 including mailing).
- ThedaStar Release of Liability Certificate
- Emergency contact form

On Your Ride-Along Day:
- Wear your uniform from your service including name badge. If you do not have a uniform, wear dark pants (no jeans), nice shirt/blouse and hard-soled shoes (boots preferred). CLOTHING APPROPRIATE FOR WEATHER. No scrubs, T-shirts, sweats, sandals, clogs or tennis shoes will be allowed. You will be unable to ride along without proper attire.
- Bring something to do. You will need something to occupy your time when not flying. You will not be allowed to station yourself in the communications center unless accompanied by a flight nurse. This is due to HIPPA / patient privacy regulations.
- Bring food or money to purchase food in the cafeteria.
- Upon arrival, report to the emergency department and let the person at the front desk know you are here for a ThedaStar ride along. They will contact the appropriate personnel for you.
- Please arrive about 15 minutes before your scheduled time.
