Why do I need insurance?

No one plans to get sick or hurt, but most people need medical care at some point.  If you don’t have health insurance you would be responsible to pay for your health care costs which could be very expensive.

How do I get insurance?

If you don’t already have health coverage through either your employer or a public program like Medicare, you can obtain insurance through the newly created Insurance Marketplace or contact an insurance agent.

What is the Insurance Marketplace?

The Insurance Marketplace is also often referred to as the “Exchange”.  The Insurance Marketplace provides the website at where you apply for health insurance.  You can either apply on-line at the website or by phone at 1-800-318-2596, 24 hours a day, 7 days a week.

What is the deadline for applying for coverage?

You cannot apply whenever you want; only during the open enrollment period for this year which started on October 1, 2013 and ends March 31, 2014.

If you don’t enroll during this period you won’t be able to obtain coverage until next year unless you have a “qualifying event”.  Common examples of qualifying events are loss of your current health coverage, a change in family due to death, birth, marriage or adoption and a location change in residence.  

Where do I go if I need help?

You can call Partnership Community Health Center at 920-882-6420 and make an appointment with a “Certified Application Counselor” who can assist you in completing your application.  Partnership Community Health Center’s Enrollment site is located at 500 W. Franklin Street, Appleton WI. 

If you cannot travel to this location please call and alternative arrangements will be made to provide you with the assistance you need.

What information do I need to enroll for coverage?

  • Social Security Card
  • Income information for every member of your household (i.e. W-2 forms, pay stubs)
  • Policy numbers for any current health insurance plans covering members of your household

What plan can I sign up for to make sure I can keep my ThedaCare provider?

ThedaCare is in the following plans:

  • Arise Health Plan – All Plans offered
  • Anthem Blue Cross and Blue Shield – All Plans offered
  • Common Ground Healthcare Cooperation – Only the “Empower” plan
