Lets Focus on Prevention - it's the Best Kind of Care

I’ve lived in the Midwest most of my life and am continually amazed at the hearts, resilience and strength of the families I meet along the way. These traits will serve you well when one of the aspects of the ACA comes into play – being fully engaged in your own care. In many ways, you already are because ThedaCare has partnered with patients for many years to provide outstanding care.
We think the focus on prevention is spot on. As much as we enjoy seeing you, we really want to see you well – not in our hospitals. We hope you feel the same way and that’s why we’re sticklers for things like mammograms, colonoscopies and annual health screenings. We also stress the importance of exercise, sleep and diet for a healthy lifestyle. The ACA takes the preventative measures we have in place to new levels.
If you have a new health plan, or your current plan took effect on or after Sept. 23, 2010, you are now covered for pre-existing conditions. You can also access many preventive services without needing to pay a copay or co-insurance, or meet your deductible (if the services are delivered by a network provider). ACA is calling these essential health benefits (EHBs), covering 10 areas of care. You can find a full list at www.thedacare.org/aca.
We make it easy for you to stay connected to your care through MyThedaCare.org, where you can send a message to your provider, check test results and more – from wherever you are. Our EMR and your After Visit Summary will keep you up-to-speed with next steps for your treatment. If you have a chronic condition, ThedaCare At Home’s remote monitoring and in-home support check up with you while you’re in the comfort of your own home. There are also onsite clinics and wellness programs from ThedaCare at Work – to keep you healthy on the job. You might say that prevention is in our DNA.
And you’ll be getting that care from the #1 physicians in the state based on overall quality in chronic disease management from the Wisconsin Collaborative for Healthcare Quality. You can rest easy knowing that ThedaCare Physicians are working to keep you healthy.
Should you need to be hospitalized, you’re covered there, as well. ThedaCare’s highly coordinated care model often means shorter stays to get you home, faster. Your doctor, nurse, pharmacist and care coordinator (who will be your helper during your stay) meet with you and your family immediately after your admission. You tell your story once and participate in planning your care.
